March 2008: Galison/Mudpuppy Recalls Wire Bound Journals Due to Violation of Lead Paint Standard

The following product safety recall was voluntarily conducted by Galison/Mudpuppy.  Consumers should stop using the recalled products immediately and contact Galison/Mudpuppy for further instructions.  Please see details listed below.

Name of Products: Wire-O Bound Journals

Units: 160 journals

Manufacture: Galison/Mudpuppy, of New York, N.Y.

Distributor: LTD Commodities LLC.

Hazard: The paint on the metal spiral bindings of the journals contains excessive levels of lead, which violated the federal lead paint standard.

Incidents/Injuries: None reported.

Description: The spiral bound journals include Mudpuppy name and the ISBN numbers printed on the back cover of the journals. Journals included in the recall are listed below:

November 2007: Spin Master Recalls Aqua Dots Due To Children Became Unconscious After Swallowing Beads

The following product safety recall was voluntarily conducted by Spin Master. Consumers should take the recalled toy away from children and contact Spin Master for future instructions. Please see details listed below.

Name of Products: Aqua Dots

Units: 132 Units

Importer: Spin Master, of Toronto, Canada

Distributor: ABC Distributing and LTD Commodities LLC.

Hazard: The coating on the beads that causes the beads to stick to each other when water is added contains a chemical that can turn toxic when many are ingested. Children who swallow the beads can become comatose, develop respiratory depression, or have seizures.

Incidents/Injuries: CPSC has received two reports over the past several days of children swallowing Aqua Dots. A 20-month-old child swallowed several dozen beads. He became dizzy and vomited several times before slipping into a comatose state for a period of time, was hospitalized, and has since fully recovered. A second child also vomited and slipped into a comatose state and was hospitalized for five days.

Description: The recalled toy is a craft kit which allows children to create various multi-dimensional designs using small colored beads. The beads fuse together when sprayed with water. The recall applies to all models of Aqua Dots. The kit includes accessories such as a 4-chamber Aqua Dot pen, 2 trays, 2 stands, 500 dots, spray bottle and 6 design templates. The product is labeled for ages 4+.

Sold by: Catalog internet from October 2007 to November 8, 2007 for $12.80.

Remedy: Consumers should immediately take the recalled toy away from children and contact Spin Master to return for free replacement beads or a toy of equal value.

August 2007: International Home Fashions and Bilt-Safe Technologies Recall "Classic Beautyrest" Electric Throws Due to Fire Hazard

The following product safety recall was voluntarily conducted by International Home Fashions and Bilt-Safe Technologies. Consumers should stop using the recalled products immediately and contact International Home Fashions for further instructions.

Name of Product: Classic Beauty Rest Electric Warming Throws

Units: 84

Importer: International Home Fashions Inc. (and sister company Bilt-Safe Technologies), of Black Mountain, N.C.

Manufacturer: Ningbo Veken Elite International Trading Company Ltd., of China

Hazard: Bunching, folding or tucking of these electric throws can cause them to overheat, resulting in smoldering, melting, fire and burn hazards.

Incidents/Injuries: Bilt-Safe has received 38 reports of the throws overheating, including at least 15 reports of fires and four reports of consumers suffering blistering or minor burns to the hand, leg and back.

Description: This recall involves 52-inch by 62-inch electric warming throws. The 100% acrylic throws were sold in various colors and patterns and have model number BST-06-THR. The model number is located on the care label sewn into the throw. “Classic Beautyrest” and “Automatic Electric Warming Throw” is printed on the product’s packaging. Model numbers with the following date codes are included in this recall: B00106 through B36506. Bilt-Safe Technologies, Inc. and the date code are printed on the bottom of the care label.

Sold at: Catalogs nationwide and on LTD Commodities LLC Web site from August 2006 through February 2007 for about $34.95.

Remedy: Consumers should immediately stop using the electric throws and contact International Home Fashions for information on where to return a portion of the power cord to receive two replacement (non-electric) throws. Consumers should not return the throws to the retailer where purchased.

June 2, 2005: LTD Commodities Announce Recall of Tree Lighted Ficus 5 feet

The following product safety recall was voluntarily conducted by LTD Commodities in cooperation with CPSC.  Consumers should stop using the product immediately unless otherwise instructed.

Units: About 2,560
Distributors: LTD Commodities, LLC of Bannockburn, IL

Hazard:
These lights attached to the Ficus Tree have undersized and exposed wires, which pose a risk of electric shock and fire hazards.

Identification: 
On the product:  At the end of the decorative light string includes a tag with the marking “E254698” and model number WL-70/2C similar to the label shown in the photograph below.












Incidents/Injuries:
None reported.

Description:
The pre-lit 5-ft Ficus Tree inside a bamboo basket includes a green light string featuring 70 clear mini bulbs wrapped and attached throughout the leaves and around the trunk.  A silver holograph UL sticker and cautionary label with UL File number E254698, Model no. WL-70/2C will be attached to the end of the cord.

Remedy:
Consumers should stop using the lights, remove the light string from the Ficus Tree and destroy the lights by cutting the cord.  All customers who received units with UL file number E254698 printed on the labels will receive new UL listed light strings around June 12, 2006 free of charge.

December 21, 2005: LTD Commodities Announce Recall of Pilates Balls

The following product safety recall was voluntarily conducted by LTD Commodities in cooperation with CPSC.  Consumers should stop using the product immediately unless otherwise instructed.

Name of product: Pilates Balls
Units: About 20,328
Distributors: LTD Commodities LLC, of Bannockburn, Ill.

Hazard: The plastic black clips at the end of the rubber tubing can come apart when in use. In addition, the grommet used to hold the black rubber tubing could separate from the nylon webbing. Consumers using the pilates balls could fall and be hit by pieces that separate.

Incidents/Injuries:The firm has received four reports of minor bodily injuries.

Description: The inflatable pilates balls are blue with black handles. They are about 26-inches in diameter when inflated. They are sold with a blue air pump.

Sold by: Catalogs nationwide from August 2005 through September 1, 2005 for about $9.

Remedy: Consumers should immediately stop using these pilates balls and contact LTD Commodities to obtain instructions on how to receive the free replacement handles. All consumers who purchased these Pilates balls were notified directly about the recall.
